Undecanoate refers to a class of chemical esters derived from undecanoic acid (an 11-carbon fatty acid). In pharmacology, undecanoate esters are used to create long-acting prodrugs by attaching the undecanoate moiety to an active molecule, slowing its release as the ester is gradually hydrolyzed in the body.

The most widely known undecanoate is testosterone undecanoate, used in testosterone replacement therapy. It exists in oral form and as injectable preparations. Orally administered testosterone undecanoate is absorbed via the lymphatic system when taken with a fatty meal, helping to bypass much of the first-pass liver metabolism. Injectable testosterone undecanoate provides a slow, sustained release of testosterone over weeks after administration, with dosing intervals typically spanning several weeks.

Pharmacokinetically, after administration the ester is cleaved by bodily esterases to release active testosterone. This prodrug approach yields more prolonged and stable plasma testosterone levels than some shorter-acting formulations, reducing the frequency of dosing in many regimens.

Clinical use centers on restoring physiologic testosterone levels in men with hypogonadism and in certain cases of gender-affirming hormone therapy, with the choice between oral and injectable forms depending on patient needs, absorption considerations, and monitoring requirements. Safety and monitoring follow standard testosterone therapy guidelines, including tracking testosterone and hematocrit levels and watching for typical androgen-related adverse effects (acne, mood changes, potential prostate effects, etc.). Use is restricted to appropriate medical supervision.

Other undecanoate esters have been investigated or used in limited contexts (for example, nandrolone undecanoate), but their availability and approval vary by country.
