Therapychemotherapy is a term sometimes used to describe the therapeutic use of cytotoxic chemotherapy to treat disease, most notably cancer. In practice, chemotherapy is a central component of systemic cancer therapy, and the phrase therapychemotherapy is rarely used in clinical guidelines. It may be encountered in discussions that emphasize chemotherapy as a treatment modality alongside surgery, radiotherapy, and biologic therapies.

Chemotherapy involves cytotoxic drugs that interfere with cell division or DNA replication, aiming to reduce tumor burden or eradicate microscopic disease. Agents are grouped into classes such as alkylating agents, antimetabolites, antitumor antibiotics, spindle poisons, and topoisomerase inhibitors. Some regimens use combinations of drugs with different mechanisms to improve efficacy and limit resistance. Administration is usually systemic, via intravenous or oral routes, but regional delivery (intraperitoneal, intrathecal) is used for certain tumors.

Indications include many solid tumors and hematologic malignancies, with uses in curative-intent regimens (neoadjuvant or adjuvant), definitive therapy for advanced disease, or palliative care. The specific agents, doses, and schedules depend on tumor type, stage, and patient factors.

Common short- and long-term adverse effects reflect damage to healthy rapidly dividing tissues: bone marrow suppression leading to neutropenia and anemia; mucositis; nausea and vomiting; alopecia; neuropathy; organ toxicity. Supportive care, dose adjustments, and monitoring mitigate risks.

Limitations include tumor resistance, toxicity, and interpatient variability. Emerging approaches combine chemotherapy with targeted therapies, immunotherapy, or radiotherapy. Strategies such as metronomic dosing, liposomal formulations, and precision pharmacogenomics aim to improve therapeutic index and outcomes.

Historically, the development of cytotoxic chemotherapy began in the mid-20th century with nitrogen mustard derivatives, leading to a broad era of drug discovery and regimens that continue to evolve.
