talonavicular

The talonavicular joint is the articulation between the head of the talus and the navicular bone in the foot. It is a central component of the midfoot and forms part of the transverse tarsal joint, working in concert with the calcaneocuboid joint to allow foot adaptability on uneven surfaces.

Anatomy and ligaments: The talonavicular joint involves a convex anterior head of the talus articulating with the concave proximal surface of the navicular. It is surrounded by a fibrous capsule and reinforced by several ligaments, including the dorsal talonavicular ligament and the plantar component of the bifurcate ligament, which extends from the calcaneus to the navicular and cuboid. The plantar calcaneonavicular ligament, commonly called the spring ligament, supports the head of the talus and helps maintain the medial longitudinal arch, indirectly stabilizing the talonavicular articulation.

Function and biomechanics: The joint provides mobility within the medial column of the foot and contributes to the overall flexibility of the transverse tarsal joint. Its movement supports pronation and supination of the foot and plays a role in adjusting the arch during stance and gait. The talonavicular joint works together with the subtalar joint to permit adaptive foot motion.

Clinical significance: Pathology can include dislocation or subluxation after trauma, degenerative arthritis, and less commonly talonavicular coalitions, which can restrict motion and affect gait. Imaging typically employs weight-bearing radiographs, with MRI or CT used for detailed assessment of bones and ligaments.
