Procoagulants, or prokoagulantit, are substances that promote blood coagulation. They act to accelerate or support the formation of a hemostatic clot by activating or enhancing components of the coagulation cascade, stabilizing platelet plugs, or providing surfaces for coagulation reactions.

Physiology and mechanism: Blood coagulation involves a cascade of proteolytic reactions initiated by tissue factor (extrinsic pathway) or contact activation (intrinsic pathway). Procoagulants include coagulation factors produced by the liver (such as II, VII, IX, and X and their activated forms), cofactors, and calcium ions. Platelets provide a phospholipid surface that concentrates enzymes and substrates, facilitating the generation of thrombin and the conversion of fibrinogen to fibrin to form a stable clot.

Clinical use and forms: Procoagulants are used to control bleeding in various settings. They include topical agents such as thrombin and fibrin sealants, absorbable hemostatic materials, and calcium-containing products. Systemic procoagulants include prothrombin complex concentrates and recombinant factor VIIa, used to treat deficiencies or to reverse anticoagulation in bleeding patients. In some cases, desmopressin is used to raise levels of factor VIII and von Willebrand factor, providing a procoagulant effect in selected bleeding disorders.

Risks and considerations: While procoagulants are valuable for stopping hemorrhage, they carry a risk of excessive clotting and thrombosis, especially in patients with predisposing conditions. Their use requires careful indication, dosing, and monitoring to balance hemostasis against thrombotic risk.
