Proinsulin is a precursor protein to insulin. It is synthesized in the beta cells of the pancreas. Proinsulin is a single polypeptide chain that is then processed within the endoplasmic reticulum and Golgi apparatus. During this processing, a specific segment of the proinsulin molecule, known as the C-peptide, is cleaved out. This cleavage results in the formation of mature insulin, which consists of two peptide chains, an A-chain and a B-chain, linked together by disulfide bonds. The C-peptide is also released into the bloodstream at the same time as insulin.

The conversion of proinsulin to insulin is a crucial step in the regulation of blood glucose levels. Insulin plays a vital role in facilitating the uptake of glucose from the blood into cells for energy production or storage. The measurement of C-peptide levels in the blood can be used as an indicator of endogenous insulin production in individuals with diabetes. This is because the amount of C-peptide secreted is equimolar to the amount of insulin produced, and C-peptide is not affected by exogenous insulin administration. Therefore, high C-peptide levels can indicate that the pancreas is still producing insulin, even if the individual is taking insulin injections. Conversely, low C-peptide levels suggest very little or no insulin production by the pancreas.
