Postdural is a term used in medicine, particularly in the context of spinal anesthesia and cerebrospinal fluid dynamics. It refers to the space or condition located outside the dura mater, which is the outermost of the three meninges surrounding the brain and spinal cord.

In the practice of spinal anesthesia, a needle is intentionally inserted through the dura mater to deliver medication into the subarachnoid space, which is inside the dura. If the dura mater is inadvertently perforated and medication or fluid is deposited outside this space, it is considered a postdural puncture. This can lead to various complications.

A common complication associated with postdural puncture is a spinal headache. This occurs when the leakage of cerebrospinal fluid (CSF) from the dural puncture site causes a decrease in intracranial pressure. The brain effectively "sinks" lower within the cranial cavity, leading to a characteristic headache that is often worse when upright and relieved by lying down.

Management of postdural puncture headaches typically involves conservative measures such as bed rest, hydration, and pain relief. If these are insufficient, an epidural blood patch may be performed. This procedure involves injecting the patient's own blood into the epidural space, which helps to seal the dural puncture site and reduce CSF leakage. Understanding the location relative to the dura mater is crucial for diagnosis and treatment of related conditions.
