Nephrofilia is a term reported in some psychiatric and sexology writings to describe a rare form of paraphilia in which an individual experiences sexual interest in kidneys or kidney-related stimuli. What is described in the literature largely consists of case reports or clinical discussions, making precise prevalence and diagnostic criteria unclear.

Clinical features typically include persistent or recurrent sexual fantasies, urges, or behaviors centered on kidneys, renal anatomy, or related medical contexts. Some accounts mention arousal linked to imagery or scenarios involving renal procedures or kidney disease, though presentation can vary. Because nephrofilia is exceedingly uncommon, associated distress or impairment is not uniformly documented and may depend on the individual and context.

Diagnosis, when considered, follows broader frameworks for paraphilic disorders. If the arousal is persistent, causes distress or impairment, or leads to unsafe or non-consensual activities, clinicians may classify the presentation under a specified paraphilic disorder in DSM-5-TR or ICD-11. Differential diagnoses include other fetishistic or body-focused sexual interests, or comorbid psychiatric conditions that might influence sexual functioning.

Treatment is not standardized due to limited evidence. Management generally aligns with approaches for other paraphilias and may include psychotherapy, particularly cognitive-behavioral therapy, psychoeducation, and, when appropriate and medically supervised, pharmacological options to reduce sexual drive (such as certain antidepressants or antiandrogen therapies). Emphasis is placed on informed consent, safety, and reducing distress or harm.
