Mesenterinen is the Finnish term referring to the mesentery, a peritoneal structure that attaches the intestines to the posterior abdominal wall. The mesentery is a continuous double layer of peritoneum that forms a fan-shaped fold, enclosing a core of adipose tissue, vessels, lymphatics, and nerves. It provides both support for the intestinal loops and a conduit for neurovascular structures.

Anatomy and variants: The mesentery proper suspends most of the small intestine (jejunum and ileum) from the posterior abdominal wall. The mesocolon connects parts of the large intestine to the posterior abdomen. In humans, the dorsal mesentery largely underpins these relationships, while ventral mesenteries contribute to certain foregut structures such as the lesser omentum and falciform ligament. The mesentery is now widely recognized as a continuous organ with a defined vascular and lymphatic core.

Vascular and nervous supply: The mesentery contains the vessels of the gut, primarily the superior mesenteric artery and its venous counterparts for the small intestine, and the inferior mesenteric vessels for portions of the large intestine. Lymphatics drain to mesenteric lymph nodes. Autonomic nerves run through the mesentery, including sympathetic fibers and parasympathetic innervation from the vagus nerve for midgut structures and from pelvic splanchnic nerves for distal regions.

Clinical relevance: Mesenteric pathology includes ischemia from arterial occlusion or nonocclusive states, mesenteric panniculitis (inflammation of the fat of the mesentery), and mesenteric cysts or internal hernias that involve mesenteric defects. Imaging and surgical concepts often emphasize the mesentery’s role in vascular supply and as a conduit for critical neurovascular structures.
