Megaloblasty refers to an abnormality in the maturation of red blood cell precursors in the bone marrow, specifically characterized by the presence of megaloblasts. These are unusually large nucleated red blood cells with a nuclear-cytoplasmic asynchrony, meaning the nucleus matures more slowly than the cytoplasm. This results in a distinctive appearance under a microscope.

The primary cause of megaloblasty is impaired DNA synthesis, most commonly due to deficiencies in vitamin B12 (cobalamin) or folic acid (folate). These vitamins are crucial cofactors in DNA replication and cell division. Without adequate amounts, DNA synthesis is disrupted, leading to the enlarged cells and the characteristic features of megaloblastic anemia. Other less common causes can include certain medications that interfere with DNA metabolism, such as chemotherapy agents, or rare genetic disorders affecting folate or vitamin B12 metabolism.

Megaloblasts are not only found in the bone marrow but can also be seen in peripheral blood smears in individuals with megaloblastic anemia. The condition manifests as a macrocytic anemia, meaning the red blood cells are larger than normal, and the total number of red blood cells is reduced. Symptoms can include fatigue, weakness, pallor, and neurological issues, particularly with vitamin B12 deficiency. Diagnosis is typically confirmed through blood tests that measure vitamin B12 and folate levels, along with a complete blood count. Treatment involves addressing the underlying deficiency, usually through supplementation.
