Innervation refers to the distribution and functional connection of nerves to a tissue or organ, enabling sensory input and motor output as well as autonomic regulation. It encompasses the central nervous system’s control and the peripheral nerve fibers that reach target tissues through peripheral nerves, nerve plexuses, or direct autonomic pathways.

Innervation is commonly categorized into somatic and autonomic components. Somatic innervation provides voluntary motor control to skeletal muscles and conveys sensory information from the skin and other senses. Autonomic innervation regulates involuntary functions, including smooth muscle activity, glandular secretion, and cardiac function, and is subdivided into sympathetic, parasympathetic, and sometimes enteric components that can operate with a degree of independence from the central nervous system.

Peripheral nerves consist of axons bundled with connective tissue and are organized into mixed nerves that carry both sensory and motor fibers. Sensory fibers typically enter the spinal cord via dorsal roots, while motor fibers exit via ventral roots, and cranial nerves provide additional routes for innervation. Communication at target tissues occurs at neuromuscular junctions with skeletal muscles or at neuroeffector junctions with smooth muscle, glands, or cardiac tissue, mediated by neurotransmitters such as acetylcholine, norepinephrine, and various peptides.

Developmentally, peripheral innervation arises from neural crest cells and placodes, which give rise to neurons and glial cells in the peripheral nervous system. Clinically, innervation can be disrupted by nerve injury, leading to sensory loss or motor weakness. Recovery may involve reinnervation and nerve regeneration, with outcomes influenced by injury location, tissue environment, and timing of intervention.
