The hypothalamus is a small but essential region of the brain, located below the thalamus and forming the ventral part of the diencephalon. It serves as a key interface between the nervous system and the endocrine system, coordinating many bodily functions through autonomic pathways and the pituitary gland via the hypothalamic-pituitary axis. By integrating signals from multiple brain regions, it helps maintain homeostasis across temperature, hunger and thirst, sleep, and energy balance.

It contains several nuclei with specialized roles. The paraventricular and supraoptic nuclei produce hormones that are released from the posterior pituitary; the arcuate nucleus and other hypothalamic groups regulate appetite and energy expenditure; the ventromedial and dorsomedial nuclei influence feeding behavior and metabolism. The suprachiasmatic nucleus acts as the central clock for circadian rhythms, while the mammillary bodies participate in memory circuits linked to the limbic system.

Endocrine control is mediated through releasing and inhibitory hormones released into the hypophyseal portal system, which regulates the anterior pituitary’s secretion of hormones such as TSH, ACTH, LH/FSH, growth hormone, and prolactin. In addition, hypothalamic neurons synthesize oxytocin and vasopressin, which travel to and are released from the posterior pituitary. Through these pathways, the hypothalamus coordinates growth, metabolism, reproduction, stress responses, and fluid balance.

Clinical significance: lesions or dysfunction can disrupt temperature regulation, appetite control, sleep, fluid balance, and hormonal balance, leading to obesity, infertility, sleep disturbances, or endocrine disorders. Because of its central role, the hypothalamus is a frequent focus in studies of neuroendocrinology and neurophysiology.
