The hypothalamic-pituitary axis refers to the functional and anatomical integration of the hypothalamus and the pituitary gland, forming a central neuroendocrine system that regulates growth, metabolism, reproduction, and stress responses. The axis communicates through hypothalamic releasing and inhibitory hormones that control the anterior pituitary and, via hypothalamic neurohormones, the posterior pituitary.

Anatomy and connections: The hypothalamus releases hormones into the hypothalamo-hypophyseal portal system, which links to the anterior pituitary (adenohypophysis). Here tropic hormones are produced in response to hypothalamic signals. The posterior pituitary (neurohypophysis) stores and releases vasopressin (antidiuretic hormone) and oxytocin, which are produced in hypothalamic neurons and transported down axons.

Hormones and targets: The anterior pituitary secretes ACTH, TSH, growth hormone (GH), prolactin (PRL), and the gonadotropins FSH and LH in response to hypothalamic peptides such as CRH, TRH, GnRH, and GHRH, with dopamine providing inhibitory control over PRL. ACTH stimulates cortisol release from the adrenal cortex; TSH stimulates thyroid hormone production; GH influences growth and metabolism; FSH and LH regulate gonadal function; PRL supports lactation. The posterior pituitary releases vasopressin and oxytocin directly in response to neural signals from the hypothalamus.

Regulation and clinical relevance: Activity is governed by negative feedback from peripheral hormones and by circadian and stress signals, linking to broader endocrine and nervous system networks. Disorders include pituitary adenomas, hypopituitarism, and other hypothalamic-pituitary conditions. Diagnosis relies on hormone assays, stimulation or suppression tests, and imaging such as MRI; treatment may involve hormone replacement, surgery, radiotherapy, or targeted medical therapies.
