Endothelin-1 (ET-1) is a potent vasoconstrictor peptide produced by endothelial cells, the inner lining of blood vessels. It is a member of the endothelin family, which also includes endothelin-2 and endothelin-3. ET-1 plays a crucial role in regulating vascular tone, blood pressure, and smooth muscle cell proliferation.

ET-1 exerts its effects by binding to two types of G protein-coupled receptors: ETA and ETB. The ETA receptor is primarily located on vascular smooth muscle cells and mediates vasoconstriction. The ETB receptor is found on both endothelial cells and smooth muscle cells. On endothelial cells, ETB receptors can promote vasodilation by stimulating the release of nitric oxide. However, on smooth muscle cells, ETB receptors can also contribute to vasoconstriction.

The production of ET-1 is regulated by various factors, including shear stress, hypoxia, and inflammatory mediators. Its release is tightly controlled to maintain cardiovascular homeostasis. Dysregulation of the endothelin system, particularly increased ET-1 levels, has been implicated in the pathogenesis of several cardiovascular diseases, including hypertension, pulmonary arterial hypertension, atherosclerosis, and heart failure. Therapeutic strategies targeting the endothelin system, such as endothelin receptor antagonists, have been developed and are used in the treatment of some of these conditions.
