Dentogingival refers to the interface and relationship between teeth and the gingival tissues. The term encompasses the dentogingival junction and the dentogingival unit, describing how the tooth surface and adjacent gingiva interact biologically and clinically. This concept is central to understanding periodontal health, restoration margins, and esthetics.

Anatomy and components. The dentogingival region includes the marginal and attached gingiva, the interdental papilla, and the gingival sulcus lined by sulcular epithelium. The junctional epithelium attaches to the cementum at the cementoenamel junction, forming a primary seal between the tooth and soft tissues. The underlying connective tissue attachment and the network of dentogingival (gingival) fibers contribute to the stability and contour of the gingiva around the tooth. The biological width, approximately 2 millimeters, comprises the junctional epithelium (about 0.97 mm) and the connective tissue attachment (about 1.07 mm) and is essential for maintaining periodontal health.

Function and clinical significance. The dentogingival unit acts as a protective barrier against microbial challenge and helps preserve tooth stability and esthetics. Disturbances in this interface—such as inflammation, gingival recession, or restorative margins that encroach on the biological width—can lead to sensitivity, pocket formation, periodontal attachment loss, or recession. Clinical considerations include preserving a healthy width of keratinized tissue, proper restoration contour, and appropriate timing of procedures that affect the dentogingival complex.

Diagnostics and management. Evaluation focuses on probing depths, tissue biotype, keratinized tissue width, gingival recession, and the integrity of the dentogingival junction. Management aims to maintain or restore a healthy dentogingival complex through plaque control, periodontal therapy, and restorations positioned to respect the biological width.
