Cryoglobulinemia is a medical condition characterized by the presence of cryoglobulins in the blood. Cryoglobulins are abnormal proteins that precipitate, or clump together, in cooler temperatures. When these cryoglobulins precipitate in small blood vessels, they can obstruct blood flow and trigger inflammation, a process known as vasculitis. This combination is referred to as cryoglobulinemic vasculitis.

The symptoms of cryoglobulinemic vasculitis can vary widely depending on the organs affected. Common manifestations include skin lesions such as purpura (bruises), ulcers, and urticaria (hives), particularly on the lower extremities. Joint pain and swelling (arthralgias and arthritis) are also frequent. Some individuals may experience kidney problems, nerve damage leading to numbness or weakness (neuropathy), and fatigue. In severe cases, it can affect the liver, lungs, or central nervous system.

Cryoglobulinemic vasculitis is often associated with underlying conditions. Hepatitis C infection is a significant cause, as is other chronic infections, autoimmune diseases like rheumatoid arthritis and Sjögren's syndrome, and certain types of blood cancers such as multiple myeloma and lymphoma. The diagnosis typically involves blood tests to detect cryoglobulins, assess for underlying causes like viral hepatitis or autoimmune markers, and kidney or nerve biopsies to confirm vasculitis.

Treatment focuses on managing the underlying cause and controlling the inflammation. For hepatitis C-related cryoglobulinemia, antiviral therapy is crucial. Immunosuppressive medications, such as corticosteroids and other agents like rituximab, are often used to reduce the immune system's activity and prevent further blood vessel damage. Plasmapheresis, a procedure to remove cryoglobulins from the blood, may be considered in severe or rapidly progressing cases. Early diagnosis and appropriate management are important to prevent long-term complications.
