Chemokine receptors are a subset of G protein-coupled receptors that bind chemokines, small signaling proteins that direct the movement of immune and other cells. These receptors regulate leukocyte trafficking, lymphoid tissue architecture, and tissue surveillance under steady-state and inflammatory conditions. They are expressed on a variety of immune cells, including T cells, B cells, monocytes, dendritic cells, and neutrophils, as well as on some non-immune cells.

Chemokine receptors are seven-transmembrane domain proteins that transduce signals primarily through heterotrimeric G proteins, most commonly the G alpha i family. Ligand binding triggers intracellular signaling cascades involving calcium flux, phosphoinositide signaling, and actin cytoskeleton remodeling, which promotes directed cell migration (chemotaxis) and changes in integrin affinity. Receptors can be desensitized and internalized via phosphorylation and beta-arrestin recruitment, regulating responsiveness to chemokines.

Classification of chemokine receptors follows the chemokine motif they primarily recognize: CCRs (c-C chemokine receptors), CXCRs (CXC chemokine receptors), CX3CR1 (receptor for fractalkine), and XCRs, among others. Their ligands are chemokines categorized as CC, CXC, CX3C, or XC. Some receptors bind multiple chemokines, and some chemokines interact with several receptors, creating a complex network that shapes cell trafficking in development, homeostasis, inflammation, and tumor biology. Notable examples include CCR5, CXCR4, and CXCR3.

Clinical relevance includes therapeutic targeting to modulate immune cell migration. Antagonists and antibodies against chemokine receptors are explored for HIV treatment (CCR5 antagonists like maraviroc), stem cell mobilization (CXCR4 antagonists like plerixafor), and various inflammatory or oncologic indications. Research continues to delineate receptor-specific roles in disease and to develop targeted interventions with favorable safety profiles.
