Canalithrelated refers to conditions or symptoms associated with the presence of otoconia, also known as "ear crystals" or canaliths, within the semicircular canals of the inner ear. These microscopic calcium carbonate crystals normally reside in the utricle and saccule, which are responsible for detecting gravity and linear acceleration. When these otoconia become dislodged and migrate into the semicircular canals, they can interfere with the normal function of the vestibular system, leading to specific symptoms.

The most common condition directly linked to canalithrelated issues is benign paroxysmal positional vertigo (BPPV). BPPV is characterized by brief, intense episodes of vertigo (a sensation of spinning) that are triggered by changes in head position. These positional changes cause the dislodged canaliths to move within the fluid-filled semicircular canals, stimulating the balance nerves inappropriately and creating the sensation of movement even when the head is still.

Symptoms beyond vertigo can include nystagmus (involuntary eye movements), nausea, and unsteadiness. While BPPV is the primary manifestation, the term "canalithrelated" broadly encompasses any disturbance or pathology arising from the abnormal location of these otoconia. Diagnosis typically involves specific positional maneuvers performed by a healthcare professional, which can elicit the characteristic vertigo and nystagmus, helping to identify which semicircular canal is affected. Treatment often involves canalith repositioning maneuvers, such as the Epley maneuver, designed to guide the displaced otoconia back into the utricle where they can be reabsorbed.
