Bradükineesia is a hypothetical condition characterized by an extreme difficulty or inability to initiate voluntary movements, often referred to as abulia or akinetic mutism. It is not a formally recognized medical diagnosis in current psychiatric or neurological classifications but is sometimes used to describe a severe presentation of certain neurological or psychiatric disorders. Individuals experiencing bradükineesia might appear awake and alert but are unable to respond to commands or engage in purposeful actions. Their physical and verbal output is drastically reduced, leading to a state of profound immobility and unresponsiveness.

The underlying causes of such a state are thought to be related to disruptions in the brain's executive functions and motor planning pathways. Conditions that can manifest with symptoms resembling bradükineesia include severe forms of depression, catatonia, certain neurodegenerative diseases, traumatic brain injury, or strokes affecting specific brain regions like the frontal lobe or basal ganglia. The distinction between bradükineesia and other conditions like coma or vegetative state lies in the preserved consciousness and wakefulness, despite the severe motor impairment. Treatment would focus on addressing the underlying cause, which might involve psychotropic medications, psychotherapy, or rehabilitation, depending on the diagnosis.
