Key methods include standardized clinical protocols, checklists (e.g., surgical safety), medication reconciliation, infection-control practices, and patient engagement. Incident reporting systems, root cause analyses, and risk assessments are used to identify and mitigate hazards.

Governance and standards are provided by health authorities, accreditation bodies, and international organizations such as the World Health Organization and IHI. National patient-safety programs promote training, data collection, and continuous improvement to reduce preventable harm.
