Antiportörer, or antiporters, are membrane transport proteins that mediate antiport, exchanging ions or molecules across a biological membrane in opposite directions. They belong to the broader family of secondary active transporters, meaning their activity uses the electrochemical gradient of one substrate to drive the transport of another in the opposite direction. The energy for transport typically comes from the gradient established by primary active transport or other cellular processes.

Most antiporters operate by an alternating-access mechanism. Substrates bind to the transporter on one side of the membrane, a conformational change exposes the binding sites to the opposite side, and the substrates are released. After release, the transporter returns to its original state to begin a new cycle. This alternating exposure enables the simultaneous movement of two components in opposite directions, often with different stoichiometries and charge movements.

Physiological roles of antiportörer are diverse. They regulate intracellular pH and cell volume, maintain ion homeostasis, and shape electrical excitability in neurons and cardiac tissue. In humans and other organisms, prominent examples include the Na+/H+ exchanger (NHE), which helps control intracellular pH; the Na+/Ca2+ exchanger (NCX), important for calcium extrusion in cardiac muscle; and Cl−/H+ exchangers of the ClC family, which contribute to pH and chloride balance. Numerous other antiporters participate in amino acid, sugar, and metabolite transport across membranes in bacteria, plants, and animals.

Overall, antiportörer represent a crucial mechanism for coordinated substrate exchange across membranes, supporting cellular homeostasis and rapid responses to physiological changes.
