Yhdistelmäinsuliini, also known as combination insulin or premixed insulin, is a type of insulin therapy that combines two different types of insulin in a single vial or pen. These combinations typically consist of a rapid-acting or short-acting insulin and an intermediate-acting insulin. The purpose of this mixture is to provide both a quick onset of action to cover meals and a longer duration of action to manage blood sugar levels between meals and overnight.

The rapid-acting component of yhdistelmäinsuliini begins to work within 15-30 minutes, peaks in 1-2 hours, and lasts for 3-5 hours. This part is designed to control the rise in blood glucose after eating. The intermediate-acting component starts working in 1-3 hours, peaks in 6-10 hours, and lasts for 10-16 hours. This longer-acting insulin provides basal coverage, helping to maintain stable blood sugar levels throughout a significant portion of the day or night.

Yhdistelmäinsuliini is often used by individuals with type 1 and type 2 diabetes who require both mealtime and basal insulin coverage but prefer the convenience of a single injection. It simplifies the insulin regimen compared to administering separate rapid-acting and intermediate-acting insulins. However, the fixed ratio of the two insulin types in a premixed formulation means less flexibility in adjusting doses for individual meal sizes or activity levels. Dosage adjustments and timing of injections are crucial for effective blood sugar management when using yhdistelmäinsuliini. It is typically administered two or three times a day.
