The term refers to two distinct movements: tilt and shift. The tilt movement allows the photographer to rotate the lens plane relative to the image plane. This capability is crucial for manipulating the plane of focus, enabling selective focus effects. This can be used to create a miniature effect, where only a small area of the image is in sharp focus, making real-world scenes appear like small models. Alternatively, it can be used to extend the depth of field beyond what is normally possible with standard lenses, allowing for sharpness from foreground to background.

The shift movement allows the lens to be moved parallel to the image plane, typically up, down, or sideways. This is primarily used to correct or introduce perspective distortion. In architectural photography, for instance, the shift function can be used to prevent converging vertical lines when shooting tall buildings from a low angle. By shifting the lens upwards, the photographer can capture the entire building without tilting the camera, thus maintaining parallel lines. This effect is often described as "straightening" the image. Tilt-shift lenses are complex and generally more expensive than standard lenses, but they offer creative possibilities and technical solutions not available with other optics.
