Thymische

Thymische is the German adjective meaning thymic or relating to the thymus, a primary lymphoid organ. In English, the corresponding term is thymic. The thymus is located in the anterior mediastinum and is most active during childhood, gradually involuting with age and becoming increasingly adipose in adults.

The thymus provides the environment for T-lymphocyte development. Immature T cells migrate from the bone marrow to the thymus, where they undergo positive selection to recognize self-major histocompatibility complex molecules and negative selection to eliminate strong self-reactivity. This thymic education shapes central tolerance and helps ensure a functional, self-tolerant T-cell repertoire. Thymic epithelial cells, dendritic cells, and macrophages participate in these maturation processes. The organ has historically been described as producing thymic hormones such as thymosin and thymopoietin, although contemporary emphasis is on the thymic microenvironment's role in T-cell maturation rather than on these hormones alone.

Anatomically, the thymus comprises a cortex rich in lymphocytes and a lighter medulla containing fewer cells and Hassall’s corpuscles. It is usually larger in children and diminishes in size with age.

Clinical relevance includes a range of thymic abnormalities. Thymic hyperplasia and thymomas are the most common thymic pathologies in adults; thymomas can be associated with myasthenia gravis, pure red cell aplasia, and hypogammaglobulinemia (Good syndrome). In DiGeorge syndrome, thymic aplasia or hypoplasia leads to T-cell deficiency. Thymectomy, the surgical removal of the thymus, is a treatment option for some cases of myasthenia gravis and certain thymic tumors. Imaging with CT or MRI is used to assess thymic size and pathology.
