Thrombin generation refers to the in vitro measurement of thrombin production during coagulation. Thrombin is the central protease of hemostasis, converting fibrinogen to fibrin, promoting clot formation, and accelerating the cascade through feedback activation of multiple coagulation factors. Thrombin generation reflects the net balance between procoagulant forces and natural anticoagulants, providing a dynamic view of an individual's hemostatic potential.

In thrombin-generation assays (TGAs), plasma or whole blood is triggered to coagulate by tissue factor and phospholipids, and thrombin activity is recorded over time. Commonly reported parameters include lag time (delay before thrombin appears), time to peak, peak thrombin concentration, and endogenous thrombin potential (ETP), the area under the thrombin-time curve. Assay conditions vary widely (e.g., tissue factor dose, phospholipid composition) and may include modulators such as thrombomodulin to evaluate anticoagulant pathways or heparin to simulate anticoagulation. Standardization remains a challenge, and results are sensitive to pre-analytical factors and instrumentation.

Clinical and research relevance: TGAs are used to investigate bleeding disorders (e.g., hemophilia A/B, von Willebrand disease), hypercoagulable states, and the effects of anticoagulants, including direct oral anticoagulants and vitamin K antagonists. They provide insight into individual thrombin-generation profiles and can help tailor therapy in some cases. However, due to limited standardization and reference ranges, TGAs are predominantly research tools rather than routine clinical tests in most settings.
