Questionnaire6 is a standardized self-report instrument developed to assess psychosocial functioning and well-being across multiple domains in adults. It comprises six subscales covering emotional well-being, social functioning, cognitive symptoms, physical symptoms, daily activities, and coping resources.

Developed by a collaboration at a health measurement consortium, it was first published in the early 2010s.
