PremiumIOLs are intraocular lenses implanted during cataract surgery or refractive lens exchange that offer advanced optical designs beyond standard monofocal lenses. They aim to improve vision at multiple distances and reduce dependence on spectacles. Premium IOLs may also correct refractive errors such as astigmatism. Selection depends on ocular health, anatomy, and patient expectations.

Common categories include multifocal IOLs, which provide simultaneous focal points for distance and near tasks; toric IOLs, designed to correct astigmatism; and extended depth of focus (EDOF) IOLs, which aim to extend a single focal range with fewer halos. Additional options include accommodating or flexible IOLs that attempt to change focus by moving within the eye, and light-adjustable IOLs whose power can be modified after implantation with controlled light exposure. Designs can be diffractive, refractive, or a combination, influencing performance at night and in contrast-sensitive tasks.

Benefits vary but commonly include improved intermediate and near vision and reduced dependence on glasses for many activities. Potential downsides include glare, halos in low light, reduced contrast sensitivity, and the possibility of residual refractive error requiring glasses or enhancements. Suitability depends on corneal health, macular condition, pupil size, and realistic expectations; some patients may still require glasses for certain tasks.

Preoperative assessment is essential, including biometric measurements, corneal topography, and evaluation of ocular surface and retina. Informed decision making involves discussing goals, potential trade-offs, and the likelihood of postoperative enhancements. Postoperative care may involve treatment for posterior capsule opacification and, in some cases, refinement of refractive outcome.
