Hypomyelinaatio

Hypomyelination refers to a group of rare genetic disorders characterized by the reduced formation or abnormal maintenance of myelin. Myelin is a fatty substance that insulates nerve fibers in the brain and spinal cord, allowing for rapid and efficient transmission of nerve impulses. In hypomyelinating disorders, this insulating sheath is either thinner than normal or not fully developed.

The underlying cause of hypomyelination is typically mutations in genes that are crucial for myelin production or maintenance. These genetic defects can affect various components of the myelin sheath, including myelin proteins, lipids, or the cells responsible for producing myelin (oligodendrocytes in the central nervous system and Schwann cells in the peripheral nervous system).

The clinical presentation of hypomyelinating disorders can vary widely depending on the specific gene affected and the extent of myelin deficiency. Common symptoms may include motor impairments such as developmental delay, hypotonia (low muscle tone), ataxia (lack of coordination), and difficulties with voluntary movements. Neurological signs can also encompass nystagmus (involuntary eye movements), seizures, and cognitive deficits.

Diagnosis usually involves a combination of clinical evaluation, neuroimaging techniques like MRI to assess the extent and pattern of myelination, and genetic testing to identify the causative gene mutation. There is currently no cure for most hypomyelinating disorders, and treatment focuses on managing symptoms and providing supportive care to improve quality of life. This may involve physical therapy, occupational therapy, speech therapy, and anticonvulsant medications for seizures.
