Hjúkrunaráætlana, often translated as nursing care plans, are a fundamental tool in the practice of nursing. They are systematic documents that outline the nursing care a patient will receive. A nursing care plan is developed by a registered nurse and serves as a guide for all healthcare professionals involved in a patient's treatment. The primary purpose of a nursing care plan is to ensure that patient care is individualized, coordinated, and effective.

The development of a nursing care plan typically follows a standardized nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. During the assessment phase, the nurse gathers comprehensive information about the patient's health status, including physical, psychological, social, and spiritual needs. This information is then used to formulate nursing diagnoses, which identify the patient's actual or potential health problems that nurses can treat.

Following the diagnoses, the planning stage involves setting patient-centered goals and expected outcomes. These goals are specific, measurable, achievable, relevant, and time-bound (SMART). The nurse then determines the interventions necessary to achieve these goals, considering evidence-based practices and the patient's unique circumstances. Implementation refers to carrying out the planned nursing interventions. Finally, the evaluation phase involves assessing the patient's progress toward the goals and modifying the care plan as needed.

Nursing care plans are dynamic documents; they are not static but are continuously reviewed and updated as the patient's condition changes. They promote communication among the healthcare team, facilitate continuity of care, and provide a legal record of the nursing interventions provided. Effective nursing care plans contribute significantly to patient safety and improved health outcomes.
