HandiHaler is a brand of dry powder inhaler designed for delivering tiotropium bromide, a long-acting antimuscarinic medication, to the lungs. It is marketed for the maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. The device is used with single-dose gelatin capsules that contain tiotropium.

The HandiHaler consists of a mouthpiece, a dust cap, and a capsule chamber. A tiotropium capsule is loaded into the chamber. When the device is closed, the capsule is pierced, and the patient inhales through the mouthpiece to deliver the powder into the lungs. The capsule is intended for a single-use dose and should be discarded after the powder has been inhaled; the device itself is reusable for multiple capsules.

Usage and dosing guidelines are as follows: for adults with COPD, the typical dose is one 18 microgram tiotropium capsule inhaled once daily. Steps include opening the dust cap, loading a capsule, closing the mouthpiece to pierce the capsule, exhaling away from the device, sealing the lips around the mouthpiece, and inhaling quickly and deeply. The capsule should not be swallowed, and the empty capsule is discarded after use. The device should be cleaned and maintained according to the manufacturer's instructions.

Safety and notes: common side effects include dry mouth, throat irritation, and sinus or respiratory infections. Tiotropium is contraindicated in individuals with hypersensitivity to tiotropium or related antimuscarinic agents. Proper inhaler technique is important for effective dosing.
