HMG-CoA reductase, also known as HMGCR, is an enzyme that plays a crucial role in the biosynthesis of cholesterol and other isoprenoids. It catalyzes the conversion of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) into mevalonate. This reaction is the rate-limiting step in the mevalonate pathway, a complex series of biochemical reactions essential for cellular function.

The mevalonate pathway is vital for the production of various essential molecules, including cholesterol, steroids, ubiquinone (coenzyme Q10), and dolichols. Cholesterol is a fundamental component of cell membranes and is a precursor for steroid hormones and bile acids. Ubiquinone is critical for energy production within mitochondria, and dolichols are involved in protein glycosylation.

Due to its central role in cholesterol synthesis, HMG-CoA reductase has become a significant target for pharmaceutical intervention. Drugs that inhibit HMG-CoA reductase, commonly known as statins, are widely prescribed to lower cholesterol levels in individuals with hypercholesterolemia and to reduce the risk of cardiovascular disease. By blocking the enzyme's activity, statins effectively reduce the liver's production of cholesterol, leading to a decrease in circulating LDL cholesterol. Genetic mutations in the HMGCR gene can lead to a rare autosomal recessive disorder characterized by severe developmental abnormalities and intellectual disability, highlighting the enzyme's importance in human development.
