Gram-positive refers to bacteria that retain the crystal violet-iodine complex after a Gram stain and thus appear purple under a light microscope. This staining property reflects a thick, multilayered peptidoglycan cell wall with high cross-linking and an absence of an outer membrane, which slows alcohol decolorization.

In Gram-positive bacteria, the cell wall often contains teichoic and lipoteichoic acids that contribute to wall structure and may influence adhesion and interactions with the host immune system. Many Gram-positive species are cocci (for example, Staphylococcus aureus and Streptococcus pneumoniae) or rods (such as Bacillus and Clostridium species). Some Gram-positive bacteria form endospores, notably Bacillus and Clostridium. Mycobacteria, while thick-walled and considered Gram-positive by some criteria, are usually identified by acid-fast staining due to their mycolic acids, which affect decolorization.

Gram-positive bacteria are contrasted with Gram-negative bacteria, which have a thinner peptidoglycan layer and an outer membrane containing lipopolysaccharide. The outer membrane and reduced peptidoglycan enable faster decolorization, so Gram-negative organisms appear pink after counterstaining.

Clinical and practical relevance is significant: many pathogens are Gram-positive, including Staphylococcus aureus, Streptococcus species, and Listeria monocytogenes. The Gram reaction helps guide initial antimicrobial choices, as Gram-positive organisms are often susceptible to beta-lactam antibiotics, though resistance (such as MRSA) requires alternative agents like vancomycin. Gram staining remains a rapid, widely used diagnostic tool that supports preliminary categorization, while taxonomic classification integrates additional phenotypic and genotypic data.
