The system assigns individuals to one of five levels, reflecting increasing restriction in functional mobility, and it has been updated with revised versions to improve reliability and widen the applicable age range.
Level I: walks without limitations.
Level II: walks with limitations.
Level III: walks using a hand-held mobility device.
Level IV: self-mobility with limitations; may use a powered mobility device or require assistance for many activities.
Level V: transported in a manual wheelchair; limited trunk and head control and require extensive assistance.
