Bicillin

Bicillin is a brand name for injectable penicillin G formulations, primarily available as long-acting benzathine penicillin G products. The two main preparations are Bicillin L-A, a long-acting intramuscular formulation, and Bicillin C-R, a combination product that contains benzathine penicillin G and procaine penicillin G for intramuscular use. Both products belong to the penicillin class of antibiotics and function by inhibiting bacterial cell wall synthesis.

Bicillin L-A provides sustained penicillin levels after a single intramuscular injection, making it useful for infections caused by susceptible organisms and for certain prophylactic indications. Bicillin C-R is a combination product designed to provide both immediate and prolonged penicillin exposure, with its components chosen to balance rapid initial activity with longer-term coverage. Indications commonly associated with these products include treatment of select streptococcal infections and the treatment and prevention of syphilis, as well as prophylaxis for rheumatic fever in appropriate clinical scenarios.

Administration and dosing: both forms are intended for deep intramuscular injection and should be administered by healthcare professionals. They are not suitable for intravenous use. Dosing varies by infection type, patient factors, and local guidelines. Inappropriate use can increase the risk of adverse effects or contribute to antibiotic resistance.

Safety: adverse effects frequently involve the injection site, such as pain or swelling, and can include hypersensitivity reactions ranging from rash to anaphylaxis. Penicillin-allergic patients require careful assessment before use. As with other antibiotics, specific safety considerations apply during pregnancy and lactation, and medical advice should be followed to determine appropriateness for an individual patient.
