Berberine is the main active constituent in these sources and is used in standardized extracts as well as in some traditional preparations.

Berberine-containing preparations are used in traditional medicine systems for gastrointestinal complaints, infectious diseases, and inflammatory conditions, and are marketed in modern times as dietary supplements for metabolic conditions. The primary biological actions attributed to berberine include activation of AMP-activated protein kinase (AMPK), suppression of hepatic glucose production, and improvement of insulin sensitivity. Additional effects reported in preclinical and some human studies include anti-inflammatory activity, lipid-lowering effects, and modulation of gut microbiota.

Clinical evidence is mixed but suggests that berberine-containing products may produce modest reductions in fasting glucose and HbA1c in people with type 2 diabetes, and reductions in LDL cholesterol in some individuals. Effects can be influenced by dose, formulation, and coexisting conditions; berberine is generally not a substitute for prescription medications. Safety and tolerability are variable: common adverse effects include gastrointestinal symptoms such as diarrhea and abdominal pain. Berberine can interact with drugs metabolized by liver enzymes and may affect anticoagulants or antidiabetic medicines; use with caution in pregnancy or breastfeeding.

Regulatory status varies by country. In many jurisdictions berberine-containing products are sold as dietary supplements with limited disease-specific claims and without formal drug approval. Quality and standardization can vary across products.
