Aldosteronedriven

Aldosteronedriven is a descriptive term used to refer to biological processes, responses, or pathologies that are primarily mediated by the mineralocorticoid hormone aldosterone. Aldosterone is produced by the zona glomerulosa of the adrenal cortex as part of the renin-angiotensin-aldosterone system (RAAS). In the kidney, aldosterone binds mineralocorticoid receptors in the cells of the distal nephron, increasing transcription and activity of epithelial sodium channels (ENaC) and Na+/K+ ATPases. This leads to enhanced sodium reabsorption, coupled with potassium and hydrogen ion secretion, resulting in increased extracellular fluid volume and, typically, higher blood pressure.

Beyond the kidney, aldosterone can affect other tissues, contributing to cardiovascular remodeling and fibrosis with chronic exposure. Aldosteronedriven processes may dominate when aldosterone signaling is upregulated or unopposed, such as in primary hyperaldosteronism or secondary hyperaldosteronism due to heart failure, liver disease, or nephrotic syndrome.

Clinical relevance includes effects on blood pressure and electrolyte balance; patients with aldosteronedriven pathology may develop hypertension and hypokalemia with metabolic alkalosis. Diagnostic approaches emphasize the assessment of aldosterone relative to renin activity, such as the aldosterone-to-renin ratio, along with confirmatory suppression or stimulation tests and imaging when indicated. Treatment targets aldosterone signaling with mineralocorticoid receptor antagonists like spironolactone or eplerenone and, when possible, addressing the underlying cause of RAAS activation.
