Tremensiksi is a term primarily used in Finnish to describe a state of delirium tremens, a severe withdrawal syndrome from alcohol. Delirium tremens, often shortened to DTs, is a medical emergency that can occur in individuals who have a history of heavy alcohol use and suddenly stop or significantly reduce their intake. This condition is characterized by a sudden and severe onset of confusion, agitation, and hallucinations.

The symptoms of tremensiksi can include tremors, particularly in the hands, which is a hallmark of the condition. Individuals may also experience disorientation, which can manifest as confusion about time, place, and identity. Auditory, visual, and tactile hallucinations are common, leading to frightening or disturbing experiences. Other symptoms can include rapid heart rate, increased blood pressure, profuse sweating, fever, and seizures. The onset of tremensiksi typically occurs 48 to 96 hours after the last drink, but can sometimes be delayed.

The exact mechanisms behind delirium tremens are not fully understood, but it is believed to be related to changes in neurotransmitter activity in the brain that occur after chronic alcohol exposure. Alcohol affects the GABA and glutamate systems, and withdrawal leads to a rebound hyperactivity of the glutamate system, contributing to the excitability and symptoms seen in DTs. Treatment for tremensiksi involves supportive care, often in an intensive care setting, and medications to manage symptoms, such as benzodiazepines to reduce agitation and prevent seizures. Nutritional support and electrolyte correction are also crucial. Without prompt and appropriate medical attention, delirium tremens can be fatal.
