Somatostatin pumps, also known as somatostatinomas, are rare neuroendocrine tumors that arise from the delta cells of the pancreas or duodenum. These delta cells normally produce somatostatin, a hormone that inhibits the release of other hormones, including insulin, glucagon, gastrin, and growth hormone. When somatostatinomas develop, they produce excessive amounts of somatostatin, leading to a specific set of clinical manifestations known as the somatostatinoma syndrome.

The symptoms associated with somatostatinomas can be varied and often include hyperglycemia due to the inhibition of insulin release, achlorhydria or hypochlorhydria resulting from suppressed gastrin secretion, steatorrhea or malabsorption secondary to the inhibition of pancreatic enzyme and bile salt release, and cholelithiasis (gallstones) as a consequence of impaired gallbladder contraction. Other potential symptoms may include weight loss and abdominal pain.

Diagnosis of a somatostatinoma typically involves elevated levels of somatostatin in the blood. Imaging techniques such as computed tomography (CT) scans or magnetic resonance imaging (MRI) are used to locate the tumor, which is often small and difficult to find. Endoscopic ultrasound (EUS) can also be beneficial. Treatment usually involves surgical resection of the tumor. In cases where surgical removal is not possible, medical management with somatostatin analogs, such as octreotide, may be used to control the excessive hormone secretion and alleviate symptoms.
