Subkostale is a term used in anatomical and clinical contexts to denote location beneath the ribs, or the region beneath the costal margins. In English medical usage the more common form is subcostal; subkostale appears in German and other languages as a corresponding adjective.

In anatomy, the subkostale region lies along the inferior thoracic cage and includes structures that run along or just under the lower border of the ribs: subcostal muscles, the intercostal neurovascular bundle (artery, vein, nerve) that accompanies each rib, and the subcostal nerve (a ventral ramus of T12). The area also corresponds to parts of the pleura and the lower thoracic wall; lymph nodes in the region may be referred to as subcostal nodes in some texts. The exact contents vary by level.

Clinically, the term informs surgical approaches and regional anesthesia. Subcostal incisions are used to access the upper abdomen or liver; injury to the subcostal nerve or vessels can cause pain or bleeding in the flank or groin. Subkostal pain can indicate rib fracture, muscular strain, or subcostal hernia, though this is uncommon.

Imaging and procedures frequently use "subcostal" in reference to views and access routes. A subcostal echocardiographic window allows cardiac imaging from below the sternum; subcostal ultrasound can evaluate the liver, gallbladder, and other upper abdominal organs. The term thus anchors anatomical, surgical, and diagnostic contexts.
