Orchiectomy is a surgical procedure that involves the removal of one or both testicles. It can be performed for various medical reasons, including the treatment of testicular cancer, management of prostate cancer, and in cases of severe trauma or infection of the testicle. The procedure can also be part of gender-affirming care for transgender women.

There are two main types of orchiectomy: unilateral, which involves the removal of a single testicle, and bilateral, which involves the removal of both testicles. The surgical approach can be either through the scrotum (scrotal orchiectomy) or through an incision in the groin (inguinal orchiectomy). Inguinal orchiectomy is often preferred for suspected malignancy as it allows for control of the spermatic cord before the testicle is removed, minimizing the risk of cancer spread.

Following a bilateral orchiectomy, a person will no longer produce testosterone, necessitating hormone replacement therapy. In cases of unilateral orchiectomy, the remaining testicle typically continues to produce sufficient testosterone and sperm. While the physical removal of a testicle is permanent, prosthetic testicles can be surgically implanted to restore a more natural appearance. The decision to undergo an orchiectomy is a significant one, and patients are counselled on the implications, including fertility, hormonal changes, and potential psychological effects.
