Metarterioles are small blood vessels within the microcirculation that link arterioles to capillary networks. They function as transitional channels that help regulate the distribution of blood flow to tissues.

Anatomy and structure: Metarterioles are typically a few tens of micrometers in diameter. They possess a layer of smooth muscle in their tunica media along much of their length, giving them contractile ability. A central thoroughfare channel runs through the metarteriole from the arterial side toward the venous side, allowing a direct arterial-to-venous path when needed. At the entrances to the capillary beds, precapillary sphincters or sphincter-like regions regulate whether blood enters the true capillaries or bypasses them. Pericytes may be associated with the walls of metarterioles and capillaries, contributing to stability and regulation of exchange.

Function and regulation: Metarterioles help balance tissue perfusion and exchange by enabling two flow options. When precapillary sphincters relax, blood passes from the metarteriole into the capillary network for exchange with surrounding tissues. When sphincters constrict, blood can pass through the thoroughfare channel directly to postcapillary venules, effectively shunting flow away from the capillaries. Local metabolic cues (such as oxygen, carbon dioxide, pH, and lactate) along with neural and endothelial factors (for example nitric oxide and endothelin) regulate this distribution.

Clinical relevance: Proper metarteriole function is important for tissue oxygen delivery and waste removal. Dysregulation of microvascular flow can contribute to conditions such as shock, metabolic disorders, and impaired tissue perfusion.
