Exemestane is a medication used in the treatment and prevention of certain types of breast cancer. It belongs to a class of drugs called aromatase inhibitors, specifically a steroidal irreversible aromatase inhibitor. Exemestane works by blocking the action of the aromatase enzyme, which is responsible for converting androgens into estrogens. In postmenopausal women, this enzyme is the primary source of estrogen production. By reducing estrogen levels, exemestane can slow or stop the growth of estrogen-sensitive breast cancers.

It is typically prescribed for postmenopausal women who have already received treatment with tamoxifen or other endocrine therapies for early-stage hormone receptor-positive breast cancer. It may also be used for metastatic breast cancer that has progressed after tamoxifen treatment. The drug is administered orally in tablet form, usually once a day.

Common side effects of exemestane can include hot flashes, joint pain, fatigue, headache, nausea, and increased sweating. Less common but more serious side effects can involve bone loss (osteoporosis) and an increased risk of fractures. Patients taking exemestane may require monitoring for bone mineral density and calcium and vitamin D supplementation. It is important for individuals to discuss potential risks and benefits with their healthcare provider. Exemestane is not recommended for premenopausal women as it can affect ovulation and fertility.
