Epididymitis is an inflammation of the epididymis, a coiled tube located on the back of the testicle that stores and carries sperm. This condition can cause pain and swelling in the scrotum.

The most common cause of epididymitis in sexually active men younger than 35 is a sexually transmitted infection (STI), such as chlamydia or gonorrhea. In men over 35, or in younger men who are not sexually active, the cause is often bacterial infections that spread from the urinary tract. Other causes can include urinary tract infections, prostate infections, certain medications, or trauma to the groin area.

Symptoms typically develop gradually and can include a dull ache or pain in one testicle, swelling and tenderness of the scrotum, and sometimes a fever. There may also be pain during urination, a discharge from the penis, or a feeling of heaviness in the testicle. In some cases, the scrotum may become very red and warm.

Diagnosis usually involves a physical examination, urine tests to detect infection, and sometimes an ultrasound of the scrotum to rule out other conditions like testicular torsion. Treatment focuses on addressing the underlying cause. If an infection is present, antibiotics are prescribed. Pain and swelling can be managed with over-the-counter pain relievers, scrotal support, and rest. It is important to seek medical attention if symptoms develop to ensure proper diagnosis and treatment, as untreated epididymitis can lead to complications such as chronic pain or infertility.
