Endothelin-1 (ET-1) is a potent vasoconstrictor peptide produced by endothelial cells lining blood vessels. It is a member of the endothelin family, which also includes endothelin-2 and endothelin-3. ET-1 plays a crucial role in regulating vascular tone, blood pressure, and cell proliferation.

The synthesis of ET-1 involves a precursor protein that is cleaved to form big ET-1, which is then further processed by endothelin-converting enzymes to yield the active ET-1 peptide. This active form exerts its effects by binding to specific receptors on target cells, primarily endothelin A (ETA) and endothelin B (ETB) receptors. ETA receptors are predominantly found on vascular smooth muscle cells and mediate vasoconstriction. ETB receptors are located on both endothelial cells and smooth muscle cells, where they can mediate vasodilation or vasoconstriction depending on the context.

Dysregulation of ET-1 signaling has been implicated in various cardiovascular diseases, including hypertension, atherosclerosis, and pulmonary arterial hypertension. Elevated levels of ET-1 can lead to sustained vasoconstriction, increased vascular smooth muscle proliferation, and fibrosis, contributing to the progression of these conditions. Therapeutic strategies targeting the ET-1 pathway, such as endothelin receptor antagonists, have been developed and are used in the treatment of certain cardiovascular disorders.
