Emakakaela is a term used in some Southern African languages, notably Zulu and Xhosa, to refer to the cervix—the narrow, lower part of the uterus that opens into the vagina. It serves as a barrier and gateway between the uterus and the vaginal canal.

Anatomy and microstructure: The cervix contains the endocervical canal, which is lined with mucus-secreting glands, and it connects the uterine cavity to the vaginal canal via the internal and external os. The tissue can be described as having an inner mucous epithelium and an outer squamous epithelium, with the junction between these tissues being clinically significant for screening and diagnosis. The cervix produces cervical mucus whose consistency changes during the menstrual cycle, influencing the passage of sperm.

Function: The cervix helps protect the uterus from infection while regulating the movement of sperm into the uterus and the discharge of menstrual blood. During pregnancy it helps keep the fetus inside the uterus, and during labor the cervix dilates to allow the baby to pass through the birth canal.

Clinical relevance: Cervical health is routinely assessed during gynecological care. Screening methods such as the Pap test and HPV testing detect precancerous changes and cervical cancer early. Human papillomavirus (HPV) vaccination reduces the risk of cervical cancer. Common cervical conditions include cervicitis, cervical polyps, cervical insufficiency, and cervical cancer.
