Weaningprotokollen refers to standardized procedures or protocols designed to guide the process of withdrawing mechanical ventilation from patients. These protocols aim to systematically assess a patient's readiness for liberation from the ventilator and to ensure a safe and efficient transition to spontaneous breathing. The development and implementation of weaning protocols have become a common practice in intensive care units worldwide, driven by the desire to reduce ventilator-associated complications, shorten the duration of mechanical ventilation, and improve patient outcomes.

Typically, a weaning protocol involves a series of steps that are initiated when a patient's underlying condition has stabilized and they meet certain clinical criteria. These criteria often include adequate respiratory muscle strength, stable hemodynamics, absence of significant fever, and appropriate mental status. The protocol then outlines the method for assessing readiness, which may involve a spontaneous breathing trial (SBT). During an SBT, the patient is placed on a T-piece or low-level pressure support for a specified duration, allowing them to breathe spontaneously.

Throughout the SBT and the subsequent weaning process, patients are closely monitored for signs of respiratory distress, such as increased respiratory rate, decreased oxygen saturation, or altered mental status. If the patient tolerates the SBT without significant distress, they may be considered for extubation, the removal of the endotracheal tube. If they fail the SBT, the protocol usually dictates a period of rest and reevaluation before attempting weaning again. Variations exist in protocols regarding the specific criteria, SBT duration, and the level of support used during weaning attempts.
