The Rh system has antigens that can cause hemolytic disease of the newborn if a Rh-negative mother is transfused with Rh-positive blood.

Compatibility testing involves cross-matching the recipient's serum with the donor's red blood cells and vice versa. This process helps identify any antibodies in the recipient's serum that could react with the donor's blood cells. If a reaction is detected, the blood is considered incompatible and should not be used.

In some cases, blood may be compatible but not ideal for the recipient. For example, a patient with a rare blood type may require a specific type of blood that is not readily available. In such situations, alternative transfusions, such as platelet transfusions or plasma transfusions, may be considered.
