Omvårdnadsprocessen, often translated as the nursing process, is a systematic, patient-centered approach used by nurses to provide care. It is a dynamic and cyclical method that guides nurses in delivering high-quality care. The process typically consists of five key steps: assessment, diagnosis, planning, implementation, and evaluation.

The first step, assessment, involves gathering comprehensive data about the patient's health status. This includes collecting subjective information from the patient and objective data through physical examinations and diagnostic tests. The information gathered forms the basis for subsequent steps.

Following assessment, the nursing diagnosis is formulated. This step involves analyzing the collected data to identify the patient's actual or potential health problems that nurses can address. These diagnoses are distinct from medical diagnoses.

The third step is planning, where nurses set patient-centered goals and desired outcomes. They then develop a care plan outlining the specific nursing interventions that will be implemented to achieve these goals. The plan should be individualized to the patient's needs.

Implementation is the action phase, where nurses carry out the interventions described in the care plan. This involves performing nursing tasks, providing education, and coordinating care with other healthcare professionals.

Finally, evaluation involves assessing whether the patient's goals have been met. Nurses review the effectiveness of the care plan and the interventions used. Based on the evaluation, the care plan may be modified, continued, or discontinued. This cyclical nature ensures that care remains responsive to the patient's changing needs.
