The most common NSAID-induced issues involve the gastrointestinal system. These can range from mild symptoms like heartburn and indigestion to more serious conditions such as stomach ulcers, bleeding, and perforation. The mechanism behind this is that NSAIDs inhibit cyclooxygenase (COX) enzymes, which are involved in producing prostaglandins. Prostaglandins play a protective role in the stomach lining, and their reduction by NSAIDs can make the lining more vulnerable to acid damage.

Beyond the gastrointestinal tract, NSAID-induced adverse effects can also impact the kidneys. Prolonged or high-dose use can lead to reduced kidney function, fluid retention, and in severe cases, acute kidney injury. This occurs because prostaglandins also help regulate blood flow to the kidneys. Cardiovascular risks, including an increased chance of heart attack and stroke, have also been associated with certain NSAIDs, particularly with higher doses and longer durations of use. Allergic reactions, though less common, can also occur. Understanding these potential risks is crucial when using NSAIDs.
