Monilääkeresistenssitilanteita, often referred to as MDR (Multi-Drug Resistance), describes a situation where a microorganism, such as bacteria, viruses, fungi, or parasites, has developed resistance to multiple antimicrobial drugs. This phenomenon poses a significant threat to public health globally, as infections caused by MDR organisms are harder to treat and can lead to prolonged illness, increased mortality rates, and higher healthcare costs.

The development of MDR is a natural evolutionary process accelerated by the widespread and often inappropriate use of antimicrobial agents. This includes overuse in human medicine, such as prescribing antibiotics for viral infections where they are ineffective, and underuse in agriculture, where antibiotics are sometimes used for growth promotion rather than treating infections. Inadequate infection control practices in healthcare settings also contribute to the spread of MDR organisms.

When an antimicrobial drug is used, susceptible microorganisms are killed or inhibited, while any resistant microorganisms may survive and multiply. With repeated exposure to multiple drugs, organisms can accumulate resistance mechanisms against several different classes of antimicrobials. These mechanisms can include enzymatic inactivation of the drug, alterations in the drug's target site, reduced drug uptake, or increased drug efflux from the cell.

Addressing MDR requires a multifaceted approach. This includes promoting responsible antimicrobial use in both human and animal health, strengthening infection prevention and control measures, investing in research and development of new antimicrobial drugs and diagnostic tools, and improving surveillance systems to monitor the emergence and spread of resistance. International collaboration is also crucial in tackling this global health challenge.
