Lepirudinilla

Lepirudinilla is a synthetic direct thrombin inhibitor. It is a recombinant hirudin analog, derived from the leech salivary gland protein hirudin. Lepirudinilla works by binding directly to thrombin, inhibiting its enzymatic activity and preventing the conversion of fibrinogen to fibrin, a key step in clot formation.

It was developed for the treatment of heparin-induced thrombocytopenia (HIT), a serious immune-mediated complication of heparin therapy. In HIT, antibodies develop against the heparin-platelet factor 4 complex, leading to platelet activation and thrombocytopenia, along with an increased risk of thrombosis. Lepirudinilla provides an alternative anticoagulant for patients with HIT who cannot receive heparin or low-molecular-weight heparins.

Lepirudinilla is administered intravenously. Its pharmacokinetics are characterized by rapid onset and offset of action, with elimination primarily occurring via the kidneys. Monitoring of the activated partial thromboplastin time (aPTT) is typically used to guide dosing and assess anticoagulant effect. Bleeding is the most common adverse effect, as with other anticoagulants. Due to its renal excretion, dose adjustments are necessary in patients with impaired renal function. The use of lepirudinilla requires careful patient selection and close monitoring to manage the risk of bleeding while effectively preventing thrombotic events.
